Community Health and Wellness Center (CHWC)

CHWC is committed to providing high quality health care and social services for individuals and families throughout their lives. Partial Hospital Program at Charlotte Hungerford Hospital

Adult Partial Hospital Program: Provides an intensive environment five days a week for people in the process of recovery from recent and severe mental health and co-occurring disorders. The primary goal is to minimize symptoms and optimize an individual’s overall level of functioning as prevention to hospitalization or re-hospitalization.​

Adolescent Partial Hospital Program: Provides comprehensive therapeutic services for mental illness and depression to adolescents (ages 13-18) and their families based on individual need and stage of recovery. The program provides an intensive therapeutic environment and partial hospital program five days a week for adolescents in the process of recovery from recent and severe mental health or co-occurring disorders. The goal of this program is to minimize symptoms with relapse prevention and optimize an adolescent’s overall level of functioning.

Homeless Outreach Program at Charlotte Hungerford Hospital

The Mental Health Services to the Homeless (MHSH) program provides homeless individuals, including families, with outreach, engagement, screening, mental health and substance abuse assessments and intakes. Other services provided to eligible homeless individuals or families are: Clinical/case management services; assistance in obtaining and coordinating social services, entitlements and income support services; housing assistance, medical services; referrals to community mental health and substance abuse programs and other services as may be appropriate.

Better Choice Gambling Program at MCCA

MCCA has dedicated resources to treat those suffering from problem gambling with the Bettor Choice Gambling Program.  This is an outpatient gambling specific treatment program funded by the State of Connecticut. Outpatient Treatment for Problem Gambling includes individual, group, budget counseling for both gamblers and family members, psychiatric evaluation for both gamblers and family members, and couple and family counseling.
